Mental Health Diagnostic & Medication Services

  • New Patient Initial Evaluation (60-75 min | $300)

    Required of new patients unless seeking gender or bariatric evaluations. Includes review of documentation, assessment, interview, diagnosis and medication as possible and warranted. Additional sessions may be required prior to diagnosis and medications.

  • Medication Management (established patients) (30 min | $150)

    Brief assessment, review of medications, brief therapy as warranted. Session length decided by provider. (titration, changes, or additions of medications)

  • Bariatric Surgery Evaluation and Letter (45-60 min | $200)

    Patients needing a psychiatric evaluation for bariatric surgery will have a complete new patient evaluation and clinical interview (unless an existing patient). Dr. Stewart may have the patient complete various assessment tools before. This comprehensive assessment includes a full written clinical report for the referring surgeon. Please allow 2 days from assessment to letter draft.

  • Emotional Support Animal Evaluation and Letter (60-75 min | $350)

    Patients seeking documentation for an emotional support animal should sign up for a New Patient Initial Evaluation appointment. Clients will have a complete new patient evaluation and clinical interview (unless an existing patient). Patients with a diagnosed mental illness should ensure that documentation is shared with Dr. Stewart prior to the appointment. Dr. Stewart may have the patient complete various assessment tools before. This comprehensive assessment includes a full written clinical report for the referring surgeon. Please allow 5 days from assessment to letter draft.

  • Cancellations / Late Cancellation Fees

    Each appointment is reserved for you. If you need to cancel or reschedule, we require at least 24-hour notice from your scheduled session time.  This allows the opportunity for someone else who is in need to benefit from the availability. If you fail to give 24-hour notice or No-Show, you are responsible for the full cash rate fee for your appointment or a minimum of $100.  If you are more than 15 minutes late for any appointment, you will be considered a No-Show and charged accordingly. For example, an appointment on Monday at 2 pm needs to be cancelled by 2pm on the Friday before. Please note that insurance companies will not reimburse for missed or canceled appointments.
